Recent payer research conducted by Xcenda in 2020 found that payers are using ICER’s UPI reports with two-thirds of payers (66%) reporting them to be somewhat to extremely useful and 61% reporting them as somewhat to extremely influential on coverage decisions. Therefore, it is important for manufacturers to be familiar with the methodological approach of these assessments to prepare for future negotiations with payers.

The restriction on copay assistance may have an adverse impact on patients’ adherence to prescribed therapy regimens and therefore may affect patient health and overall healthcare costs. While large, self-funded, employer-sponsored plans have led the charge in saturating the commercial market with these designs, recent federal regulation has given way to even more plans adopting AAPs and threatened the beneficial impact of copay assistance to patients on a long-term basis. The framework for the Food and Drug Administration’s (FDA’s) RWE Program, published in 2018, serves as a roadmap for the inclusion of real-world data (RWD) and RWE in regulatory decisions, including standards on how RWD are defined, collected, and analyzed. This article reviews the use of RWE in United States (US) regulatory decisions after the publication of the FDA’s RWE framework and describes how manufacturers can leverage RWE to optimize their product launch. Unfortunately, there is no magic bullet to improve outcomes. Many of the first-line prescription drugs to lower risk factors for ASCVD are low-cost generics and thus have broad coverage among payers. Despite the easy access to high-value drugs, there are persistent gaps in adherence. Other patients with adverse outcomes simply do not respond—or respond well—to the available therapies, despite fully complying with their physicians’ instruction. This biologic basis for a less-than-average response shows the need and opportunity for continued innovation to develop treatments for the spectrum of people with ASCVD.
